Youth Ministry Registration
CATHOLIC YOUTH MINISTRY REGISTRATION FORM
To be a part of the youth ministry program at St. Francis of Assisi-St. Blaise Parish , parents or guardians of a minor (youth under the age of 18) and young people who are 18 and above will have to fill out a catholic youth ministry registration form. The form can either be completed online and sent to stfstbyouthministry@gmail.com or downloaded and completed manually. If the latter option is chosen, you may submit to the Youth Ministry Director, Elaine Tontoh or to the rectory during office hours. Only those who are registered may participate in Catholic Youth Ministry. Please send any enquiries to the email address above.

COMBINED REGISTRATION, CONSENT & HEALTH FORM FOR MINORS (YOUTH UNDER 18)
Every youth under the age of 18 is required to complete this consent form for specific events organized by the youth ministry off-premises. The forms have to be signed by a parent or guardian. The form can either be completed online and sent to stfstbyouthministry@gmail.com or downloaded and completed manually. If the latter option is chosen, you may submit to the Youth Ministry Director, Elaine Tontoh or to the rectory during office hours. Every youth ( aged 18 ) and young adult (above the age of 18) who wishes to participate in an event being organized by the youth ministry off-premises needs to complete and sign a consent form.
